COVID-19 e diabetes mellitus: uma revisão sistemática acerca de complicações, marcadores inflamatórios e mortalidade

Resumo

A COVID-19 acomete principalmente indivíduos que apresentam comorbidades pré-existentes, dentre elas o Diabetes Mellitus (DM). Essa revisão sistemática objetivou estudar as relações entre COVID-19, DM, inflamação, complicações e desfechos. Realizou-se busca nas bases de dados MEDLINE e SCOPUS, até 05/05/2021. Selecionaram-se estudos observacionais contendo: pacientes de ambos os sexos com diagnósticos de DM e de COVID-19 e descrição de provas inflamatórias. Incluíram-se 35 estudos. Os participantes com DM apresentaram maiores níveis de marcadores inflamatórios, maior prevalência comorbidades cardiometabólicas e tendência a idade mais avançada quando comparados àqueles sem DM, o que se associou a proporções significativamente maiores de complicações da COVID-19, de comorbidades, de admissão em UTI, de duração de estadia hospitalar e de mortalidade na amostra. Conclui-se que os marcadores inflamatórios possuem valor preditor negativo no prognóstico dos pacientes que possuem COVID-19 concomitantemente com DM e que possivelmente refletem a sobreposição de mecanismos fisiopatológicos das duas doenças.
